Amit R. Chokshi, M.D.
A board-certified ophthalmologist, Dr. Amit Chokshi is highly trained in laser vision correction and is graduate of the University of Miami’s Miller School of Medicine. After completing his residency at Vanderbilt Medical Center, where he began performing refractive eye surgery, Dr. Chokshi was awarded a fellowship at the Laser and Corneal Surgery Associates/New York Eye and Ear Infirmary, where he specialized in refractive and corneal surgery.
Dr. Chokshi went on to serve as the director of the University of Florida Laser Eye Center. He also held the positions of assistant professor and director of the cornea and external disease division at the UF Department of Ophthalmology. Dr. Chokshi is a member of the American Academy of Ophthalmology, American Society of Cataract and Refractive Surgery, the International Society of Refractive Surgery, the Tear Film and Ocular Surface Society, and the Cornea Society.
Gerard A. Coluccelli, M.D.
With more than 25 years of ophthalmic practice in the Jacksonville area, Dr. Coluccelli has established himself as one of northern Florida’s premier ophthalmic surgeons. He graduated from McGill University in Montreal and earned his doctorate at State University of New York in Syracuse. After serving as Chief Resident of Ophthalmology at Upstate Medical Center in Syracuse, Dr. Coluccelli was awarded a prestigious fellowship at Azar Eye Clinic in New Orleans. While in New Orleans, he also served on the faculty of Tulane Medical Center as a part-time clinical instructor of ophthalmology. In 1992, he co-founded the Baptist Eye Institute and has served as Chairman of the Department of Ophthalmology at Baptist Hospital in Jacksonville.
Dr. Coluccelli is board certified by both the American Board of Ophthalmology and the National Board of Medical Examiners, and has received numerous accolades for his work in laser vision correction. He is a member of several professional organizations, including the American Society of Cataract and Refractive Surgery and the International Society of Refractive Surgery. A veteran of thousands of LASIK procedures and a LASIK patient himself, Dr. Coluccelli was the area’s first to utilize Orbscan technology, which takes ophthalmic diagnostics to the next level.
Hanaa M. Habashi, O.D.
A native of Freemont, California, Dr. Hanaa Malik Habashi earned her undergraduate degree from the University of California at Berkeley. She received her Doctor of Optometry degree from the Pennsylvania College of Optometry. She continued her optometric training through residency at Bascom Palmer Eye Institute in Miami, Florida, specializing in ocular disease.
Dr. Habashi has extensive experience in refractive co-management, family eye care, including pediatric eye examination, pathology, low vision consultation and contact lens: rigid gas permeable, Keratoconus, toric and multi-focal fitting.
Florida State licensed and board certified, Dr. Habashi’s training and experience allow her to diagnose, treat, and manage or co-manage all eye conditions. She has cared for patients as an optometrist in the Jacksonville area since 2002.
Professional memberships include the American Optometric Association, Florida Optometric Association, Northeast Florida Optometric Society and the Florida Department of Education: Rehabilitation Center for the Blind.
